About Jenny Reed
Jenny Reed is the Vice President of Comprehensive Care Management for Baylor Scott and White Health (BSWH), the largest non-profit health system in Texas. Ms. Reed oversees care management departments in hospitals throughout the System as well as Baylor Scott & White Quality Alliance (BSWQA).
Jenny’s team is made up of experienced RN care managers, health coordinators, licensed social workers, community health workers and coders. Together, they are responsible for managing the care of patient populations including complex disease management, gaps in care services, psychosocially complex case management and facilitating patient continuity of care before, during and after hospitalization.
As the head of Comprehensive Care Management across BSWH, Ms. Reed leads a vital element of population health. She sets the strategy for outreaching the 43,000+ high and rising risk patients in BSWQA and building other initiatives like HealthAccess navigation services and clinical documentation improvement.
In addition to her care management responsibilities, Jenny directs funding projects focusing on healthcare strategic improvement for BSWH and Post-Acute Care strategy for BSWQA. Ms. Reed is a Louisiana native who earned her Bachelors of Science degree from the University of Louisiana at Lafayette and holds a Masters of Social Work from Louisiana State University.
