Medication Management
A detailed account of all medications is completed to ensure that you or a loved one is not taking duplicate medications and to avoid mixing medications that should not be mixed. Medication education is provided to you or your caregiver with instructions on set up and administration in order to remain independent with at home whenever possible.
IV Medication Management
Our medical team will draw labs in the home based on physician orders and provide patient and caregiver education on how to safely administer IV medications and antibiotics. IV dressings will be changed out weekly in your home by our experienced nursing staff.
Anticoagulation Therapy
We provide patients in-home monitoring of anticoagulants, medication education, nutrition recommendations and exercise routines that aid in anticoagulation therapy.
