As a premier provider of therapy services, Baylor Scott & White Institute for Rehabilitation is proud to provide you with the high-quality care you trust when and where you need it. Our goal is to get you back to better, helping you return to your everyday activities. Now, with Outpatient at Home, you have the option of being treated in the comfort and convenience of your own home. Providing care in a healing and positive environment is something we are passionate about and ensure for all who are referred to us.

For our patients who may be unable to travel to one of our outpatient clinics, we bring the care you need right to your doorstep. Our physical therapists bring all the necessary equipment to your home to optimize your treatment. Our licensed physical therapists review your needs and goals and develop an individualized care plan. We accept most major insurances and our patient service specialists will work with you to verify your coverage and answer any questions.

What you can expect with Outpatient at Home

  • Personalized treatment: Your therapist will tailor a care plan to your needs and track your overall progress.
  • Home exercises: Your therapist will prescribe exercises for you to perform between sessions that will support your progress and help you meet your goals.
  • Clear communication: Your therapist will follow up with you between appointments to ensure you’re doing well.
  • Get you back to better: Your experienced physical therapist will work with you all the way to the finish line.

Following in-home care, there may be a seamless transition to a convenient Baylor Scott & White Institute for Rehabilitation – Outpatient Therapy center to continue the healing journey.

Referring a patient to Outpatient at Home

  • Post-operative therapy but do not have transportation to a traditional clinic
  • After-hours or weekend treatments
  • Therapy but do not have childcare
  • Therapy but do not have transportation however, are not homebound
  • Therapy for any other reason they might not be able to come to one of our traditional clinic settings

Care in the comfort of your home

Following an injury or illness, some patients may be unable to travel to or participate in continuing outpatient therapy. Baylor Scott & White Institute for Rehabilitation – Home Health Services offers highly specialized therapy and skilled nursing services right in the comfort and convenience of your own home.

This program is designed to advance your recovery, helping to restore your skills and optimize your independence. Our experienced team of licensed physical, occupational and speech therapists and nurses will establish a treatment plan based on your individual needs and rehabilitation goals.

Together, we will work to improve your physical and functional abilities in the familiar surroundings of your home, rather than a nursing or other care facility.

To be eligible for home health, you must be under the care of a physician who has determined that your health condition requires you to be homebound. A doctor must also certify that you are in need of skilled care in one or more of the following: intermittent skilled nursing, physical therapy, speech therapy or continued occupational therapy.

Skilled nursing services

Our nursing team provides personalized care to effectively address a wide range of medical issues, including:

  • Gastrostomy/PEG tube care
  • Foley catheter care and training

Therapy services

Our goal, like yours, is for you to continue to progress as quickly and safely as possible. Our licensed physical, occupational and speech therapists expertly evaluate and tailor treatment to address your needs, including:

Teaching and assessment

Education is an important part of your care. To help you become more independent, our Home Health team uses the Texas Medical Foundation (TMF) teaching and assessment materials that include:

  • Hospital risk assessment tools
  • Patient emergency plan
  • Disease-specific zone teaching
  • Education curriculum for patients and families
  • Education to reduce re-hospitalizations

Other Home Health services

An on-call nurse is available 24 hours a day, seven days a week to assist you if needed. In addition, home health aides and medical social workers are a part of your Baylor Scott & White Institute for Rehabilitation Home Health team.
