Referring a Patient for Admission

Patients are typically referred to Baylor Scott & White Institute for Rehabilitation by their physicians or discharge planners (social workers, case managers) at an acute care hospital or other health care facility where they are being treated. In some cases, patients may come to us directly from home.

To facilitate admission to our campuses, please contact the respective admission office at one of our locations below.
The following documentation is required:

  • History and physical
  • Physician and nursing notes
  • Physical and occupational therapy notes
  • Medications
  • Lab reports
  • Fact sheet

A Baylor Scott & White Institute for Rehabilitation physician will carefully review your medical records and determine whether admission is appropriate for you based on your medical history, rehabilitation needs and services required. Your ability to participate in and benefit from three hours of therapy a day (or 15 hours a week, as tolerated) and achieve individual rehabilitation goals will also be reviewed. Patients admitted to Baylor Scott & White Institute for Rehabilitation must also require at least two of the following treatment disciplines: physical, occupational and/or speech therapies.
