Everyone at Baylor Scott & White Institute for Rehabilitation – from the clinical liaisons who may meet with you at the hospital, to the admission teams that help you get settled and the rehabilitation specialists that provide your care and treatment – is focused on providing you with an exceptional rehabilitation experience.

In order to begin treatment, we’ll share information with you and ask you to sign a few forms. Patients are encouraged to have a family member and/or legal guardian present to help expedite this process. Please be sure to bring copies of your insurance cards so we can confirm coverage and information. We also ask that you provide copies of your Advance Directive (Living Will) and Power of Attorney, if applicable.
